Our new patient forms can be downloaded and completed in the convenience of your own home.

New patients often remark how “homey” our practice is. That’s good, because our goal is to offer state-of-the-art chiropractic care in a comfortable surrounding. So, make yourself at home!

Make Yourself at Home

You’ll find plenty of convenient free parking and a well-lit parking lot across the street at the Park Ridge Library. We are located on the second floor of the historic landmark “Pickwick” Building.

When entering our practice you will be greeted by the patient coordinator – and if you’ve already completed your admitting paperwork, great! If not, after doing so we might show you a short 5-minute video and then you’ll meet Dr. John.

We Explain Everything First

The purpose of your first visit with the doctor is to discuss your health goals and see how the doctor may help you. Therefore, if it looks promising a thorough examination will follow to help identify the “cause” of your problem.

Since the 1st visit requires both a consultation period and a thorough examination by the doctor we request you schedule an entire hour.

The initial cost of the 1st visit which includes a consultation period along with the doctors examination is $195.00. This does not include (if required) the cost of any blood tests, urinary analysis or prescribed nutrients.
